First Visit

Brief Paperwork

Upon entering the office, we have some paperwork for you to fill out. The paperwork provides us with general information about yourself and your condition, as well as details about your overall health, medications, or past surgeries. We prefer to have a complete health history to serve you better. You can download and print these documents and complete them at home. Please fill them out as completely as possible and bring them with you to your first visit.

New Patient Paperwork

If you were involved in an Automobile accident please fill out the accident history form below in addition to the above forms.

  1. Accident History

Consultation

Next, you will have a consultation with Dr. Lare Ziemba to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Lare Ziemba to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals

History and Examination

A physical exam will be performed, including vital signs, orthopedic and neurological tests, and range of motion in order to determine what your diagnosis is and if chiropractic can help you. These tests help to determine if your pain, condition, or problem of concern is due to a musculoskeletal problem- like a disc or arthritic joint damage, versus perhaps a systemic problem like referred pain which may be from an organ.

X-ray Studies

Depending on your specific condition, we may take X-rays as well. We have a digital x-ray machine on site. X-rays help us develop the most effective treatment plan for you, and alert us of any serious spinal or bone conditions. X-rays also identify spinal misalignments and disc or joint degeneration and genetic conditions you may not have been aware of.

Same Day Treatment

Once finished with the examination and all appropriate studies, patients will generally be provided with their first treatment during this same visit. This may include spinal adjustments, physical therapy modalties, and/or soft tissue treatment.

Home Instructions

Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.

Schedule Your Next Appointment

If we feel you condition can be corrected or treated successfully with chiropractic care, you will be given a plan of care, and the staff will assist you in sceduling your appointments for your follow-up visits. Treatment options and expectations for your condition will also be discussed during this time.
